ARM POSITION CAN MODULATE HUMAN VISUAL FIELD LOSS

Damage to primary visual cortex typically results in permanent visual loss, alt
hough recent evidence suggests that limited improvement may be possible with tr
aining. We report a case that suggests that human primary visual loss may be mo
re remediable than previously thought. After a PCA infarct that included primar
y visual cortices in the right hemisphere, patient WM described a "hole" in his
 vision. This hole appeared to be smaller when he self-probed his visual field
with his own hand than when he was tested by someone else or via computerized p
erimetry. Motivated by single unit recordings in monkeys (Graziano, Yap, & Gros
s, 1994) that have described premotor neurons with arm-centered visual receptiv
e fields, we hypothesized that WM's arm position attenuated his visual loss. In
 a series of detection tasks, we first mapped WM's visual fields thereby defini
ng the region where he was unable to detect targets. This confirmed a nearly co
mplete left visual field cut. We then retested WM with his left arm placed out
in front of him, his hand resting alongside the left edge of the computer scree
n. In this condition, he was able to detect targets in previously "unseen" loca
tions in his left visual field. As predicted, target detection returned to base
line when his hand was placed back into his lap. WM's performance did NOT vary
with arm position when targets were presented out-of-reach (150 cm). We suggest
 that arm position is modulating WM's visual field loss in near space. The data
 are consistent with previously reported dissociations between near and far spa
ce in humans and provide an important link between human vision and reports of
arm-centered receptive fields in monkey premotor neurons.
